To be eligible for B.O.C.K. membership, herein after known as "the Club", you must read and sign this form. Successful completion of this form along with proof of age shall be submitted before guests may participate in club events.
By signing this form, and acknowledging this waiver, I hereby certify I am over twenty-one (21) years of age. I express full understanding that I am responsible for my consumption of alcoholic beverages. I agree to drink responsibly at any Club-sponsored event, including, but not limited to, a Club meeting. Regarding my personal transportation to and from any Club sponsored event, I agree to only use safe transportation as defined by the laws and regulations of the jurisdiction in which the event is held. I understand I am the liable party and hereby assume all liability for myself. I understand the Club as a corporation and/or its officers, or meeting host(s) whether as representatives of the Club or as individuals, assume no liability for any negligence which may arise at any Club sponsored event. This includes but not limited to, any negligent action that may arise from my consumption of alcoholic beverages at any Club sponsored event.
